Pay Per-visit - $200/per-visit ( Non-members) This price point includes the visit and E-scribed prescriptions (additional pricing on testing and medications)

Urgent Care Telehealth - $65.00/Per-Video visit ( Non-members )

Essential Wellness - $100 /month  Healthy adults wanting basic support 4 visits/year • unlimited Messaging • Annual wellness labs *Telehealth

Core Wellness - $120/month  Most patients — balanced approach 6 visits/year • Same/next-day scheduling • Comprehensive labs *Telehealth

Total Wellness - $200/month HRT, peptides, deeper lab work monthly visits • Hormone + peptide oversight • Advanced panels *Telehealth

Family Plan - $400/month Up to 2 adults + 2 kids under 18 Core Wellness services for all members • Family-focused care *Telehealth

Elite Concierge - $300/month Executive Unlimited office visits • Full functional labs • Concierge / home scheduling • 1:1 health planning *Telehealth

Home Town Heroes - $65/month Includes Annual Visits & discounts on Labs, unlimited Telehealth, and Urgent Care in office fee

** All of they above have access to Urgent Care through Telehealth, as well as in office. (in office UC subject to additional transparent fee's for procedures)
